School Nurse Update Reminder
We have a new email!
To contact the school nurses please email
4386-nurse@schools.vic.edu.au
This is a centralized account that all of our nurses have access to, and can see any plans or information that is sent through here.
Anaphylaxis/Asthma/Allergy Plans
Please scan and send to the nurse email.
Plans need to be updated on our system at the beginning of each school year or replaced prior to the review date on the plan and signed by the Doctor.
ALL plans are to be completed by the child’s Doctor (GP/Pediatrician/Specialist) and signed/dated at each review.
Medication for use during school hours or activities:
- in original packaging with the student's name clearly marked
- with the name, dose and expiry date on the medication
- with at least 6 months left to use
- if there are any special storage instructions these should be completed on the Medication Authority form
- completed and signed Medication Authority form with time/dose/medication/dates from-to and reason to be given
- please keep a reminder in your personal calendar so you can stay on top of the expiry dates for your children’s medication
Medical updates/Changes to medical care
If your child is diagnosed with a condition or their current diagnosis changes, please email us an update and any new plan/medical letter to keep on file.
Camps and Excursions
We will be sending out Camp Medication forms with all of the upcoming camp information.
Medication forms and medication are to be returned to the Nurse or classroom teacher 1 week prior to camp so we can check and ensure everything is accounted for.
Please refer to the above medication instructions about how medication should be delivered.
Failure to have medication delivered in original packaging and labelled correctly will mean it will not be dispensed.
Any questions please email us and we will respond accordingly.
Kind regards,
Lindsey Booth EEN, Sheree Bollom RN, Carolyn Ridley RN.
